Legal Business Name: Enter the legal name by which the registrant is incorporated and pays taxes. The legal business name as entered must match the legal business name in the Dun and Bradstreet registration.

Doing Business As (DBA): If the registrant commonly uses another name, such as a franchise, licensee name, or an acronym, enter that in the DBA field. The DBA name must also be listed in your Dun and Bradstreet registration. Do not enter "same" or "as shown above" in the DBA field.

Division Name: If the registrant is a division of a larger company, enter the name of the division.

You must identify the registrant as a U.S. or foreign registrant, since other requirements are based on that information.

U.S. Taxpayer Info: This application may not be used to enter the Social Security Number (SSN) for an individual or sole proprietorship. An Employer Identification Number (EIN) may be entered for a sole proprietorship, and is required for other U.S. companies. The EIN may be entered with or without the dash after the second digit.

A 9-digit DUNS Number is required for all U.S. registrants except individuals. If the registrant is registered in the System for Award Management (SAM), you may enter the DUNS number and then click the 'Search SAM' button above to display public information from the SAM registration in another window, to use as a reference from which to copy other information requested here.

DUNS Plus 4: If the registrant has a DUNS+4 number, enter that 4-digit number.

Parent DUNS Number: If the registrant is the subsidiary of another company, enter the parent company's DUNS number.

Physical address is required. If a DUNS number was supplied, the address will be validated against the company's Dun and Bradstreet registration. For U.S. addresses the full 9-digit ZIP+4 code is required.

State/Province/Region values are dependent on Country values, so select a country first. Our SAP system requires a region value if any are listed in SAP for a foreign country.

PO Box address is optional. If a PO Box is entered, do not enter the words 'PO Box'; only enter the number.

Please enter the company's primary telephone number, mobile phone number, FAX number, and email address, as applicable. All of these fields are optional.

Enter the requested information for the person who acknowledges that the information provided in the registration is current, accurate, and complete. All fields except FAX number are required.

The person listed will be contacted if we have questions about the registration and will be notified of its final disposition. If the registration is approved, this point of contact will be notified to provide required tax documentation and EFT information. Payment on purchase orders and/or subcontracts will be withheld until proper tax documentation is provided.

Type of Business/Business Structure: Business Structure values are dependent on Type of Business, so select Type of Business first.

Fiscal Year End Month is required for all registrants except U.S. individuals, for whom December is assumed to be the correct month unless a different month is selected.

Average Annual Revenue/Average Number of Employees.  The Consolidated Appropriations Act of 2014, Title III, Section 318 authorizes DOE to count 1st tier subcontracts awarded by its M&O contractors to small businesses toward accomplishments of its annual small business goals.  The following data is required for this reporting.

Average Annual Revenue

This is the average receipts over the last three completed fiscal years1.  Including receipts of the parent organization, all branches, and all affiliates worldwide, and not just the size data associated with the entity location represented by the specific DUNS number you are registering.  This information may be used to review your business size using SBA's official size standards for your industry.

(1Average reported receipts will be based on your organization's fiscal year.)

Average Number of Employees

This is the average number of employees including all affiliates.  The average number of persons employed for each pay period over the firm's lastest 12 months, including person employed by the parent organization, all branches, and all affiliates worldwide.   This information may be used to review your business size using SBA's official size standards for your industry.

U.S. small-business data entered here will be validated against the registrant's SAM registration, where small-business data is derived from the U.S. Small Business Administration (SBA). SBA 8A Certification Expiration Date is required only for 8A firms.

North American Industry Classification System (NAICS) Codes are a method for classifying business establishments. You must supply at least one for the registration to be complete. Enter only 6-digit NAICS codes in the registration.
